Incident Reports and When to Use Them

Incident reports are used to report behavior witnessed by an official that occurs OFF OF THE ICE by non-participants of the game. Fans ejected for zero tolerance violations, fans fighting in the stands, officials confronted and/or verbally abused off of the ice surface by anyone and threats made to an official leaving the building are some examples of things that should be reported on an Incident Report Form.

This report is to be used to inform the AHAI Rules & Ethics Committee of any incidents that occur at an USA Hockey sanctioned game that involves spectators, rink personnel, players, coaches, minor officials, team officials or any other people whose actions are not covered by the Playing Rulebook. Such incidents might involve verbal or physical abuse of an official, an altercation between spectators, confrontations in a parking lot or anything else that is detrimental to the game of amateur hockey and displays poor sportsmanship.
