With the AHAI assigning site going away, we currently do not have a reporting system for rink incident reports until Zebra Secretary is launched. As a temporary solution, we have created this form to report any rink incidents you may encounter.
Please remember, this form is only to be used for the following issues:
- Concussion Sympton(s) Observed
- Missing CEP Information on a coach
- Physical Attack
- Zero Tolerance
- Other
This form does not replace AAU, ACHA or USA Hockey game reports for penalties. Those should still be submitted through their existing platforms. This form only replaces the former AHAI incident report.
